Easy Surgical Approach of the Posterolateral Corner of the Knee

Background: The anatomy of the posterolateral corner (PLC) knee is complex. approach PLC can be a challenging and stressful surgical time. Indications: indications are meniscal repair, open lateral meniscus allograft transplantation, tibial plateau fracture, reconstruction for grade III sprains. Technique Description: skin incision straight, realized with positioned at 90° flexion, passing slightly posterior to epicondyle, anterior fibular head (FH), ending on Gerdy’s tubercle. subcutaneous tissues dissected posteriorly so as expose FH biceps femoris (BF) tendon. aponeurosis peroneus muscles incised vertically opposite side FH. common nerve exposed neck fibula. Metzenbaum scissors then inserted subaponeurotically, posteriorly, parallel BF tendon, superficially nerve. An made scissor’s blades, freeing tendon spread forward gastrocnemius pulled posteriorly. in closed position between joint capsule, create triangular door proximal base. base consists gastrocnemius, capsule. A counter-angled Hohmann retractor now applied against retract medially, exposing knee. Results: Noble structures easily protected. reclined popliteus vessels protected by gastrocnemius. Passing under allows better vision along less constraint than above, working window further away from femoral insertion Discussion/Conclusion: present simple, safe, reproducible exposure
